Peroneal tendon injuries and anatomic variants

Abstract

Introduction: Peroneal lesions range from tenosynovitis to complete ruptures. Anatomic variants inthe retromalolar sulcus, presence of accessory muscles, hypertrophy of the peroneal tubercle, distalextension of the peroneus short muscle, cavo-varus etc, may predispose to the development of thelesions. Methods: Retrospective post-surgical follow-up of 43 patients (27 men, 16 women) foran average of 51.9 (12-151) months. Mean age 30.9 (16-67 years), evaluating peroneal lesionsand presence of anatomical variants that predispose to the development and persistence of thesame, surgical treatment and possible complications. Results: Forty-three patients evaluated, 38of the peroneus brevis, 13 of the peroneus longus, all of them could establish the presence of atleast one anatomical variation in the osteo-fibrous course of the tendons until their distal insertions.There were 9 complications, 2 of them permanent. One patient unsatisfied by a persistent varus ofthe hindfoot, the rest totally satisfied. Conclusions: When approaching a patient with a peronealtendon injury, we must consider the treatment of their tendon alteration, as well as the resolutionof the anatomical alterations present related to the development and persistence of the pathologicalprocess, in the search for an optimal result.
