Xanthomas benign tumors are slow-growing and can affect the tendons as rarest form. We report a case of a 36 years old patient with bilateral Achilles tendon xanthoma and need total resection by the size of the tumor, with pain on moderate physical exercise. It was necessary to reconstruction with the fibular brevis and the plantaris longus tendons transfers with reinforcement by Vulpius technique. Postoperatively the patient kept with cast immobilization for 12 weeks. After removal of the plaster the patient underwent physical therapy rehabilitation, developed with good muscle strength recovery of the Achilles tendon and good mobility to date.
