A resurfacing implant and a set of instrumentation “patient specific” manufactured according to preoperative 3D virtual surgical planning based on advance CT scan images processing to treat a massive osteochondral lesion of the talar dome with failed previous surgery is present. Resurfacing implants has gained a place in this scenarios because its efectiveness in pain control. Virtual 3D surgical planning described here allowed an accurate restitution of the articular surface defect, key to success due to the geometrical and biomechanical complexity of the ankle joint. Likewise the possibility of having a “patient specific” set of instrumentation greatly simplified the execution of the procedure.
