Some children with neurological disorders presents club foot at birth which constitutes a complex condition because of the severity of the deformity and difficulty for treatment. The neurological club foot is caused by a neuromuscular imbalance and here the astragalectomy has its indication as a primary procedure or after the failure of a posteromedial release technique. During the period between Ja n ua ry 2005 a nd Decem ber 2008, we made the resection of the astragalus in 10 patients (12 feet), with rigid and severe neurological equinovaros feet. The d iag nosis was myelomen ingocele in seven patients, multiple congenital artrogryphosis in two patients and static encephalopathy in the rernaining patient. Six patients were male and four female, the average age at the moment to realize the procedure was 8 years 7 months (range:5 years - 12 years). The average follow-up was 2 years and 1 month (range: 6 months - 4 years). The technique was made as primary procedure in 2 patients (4 feet) and as treatment of recid iva in B patients (11 feet). Five cases were bilateral and five unilateral.
